Got her back today.
You know, the more I think about this issue the more it really really bothers me.
That crappy o-ring clamp is entirely wrong for a shifting sequence that requires you to lift up on the ****. There it really nothing holding the **** on the shaft other than the tension of that ring/nut stupidity. All the grip there is on the sides of a smooth round shaft. Why the heck didn't they just use a normal screw-thread. There would be no problems, and great verticle stability.
I am starting to think this will happen to everyone, over and over again.
