It is quite possible that the brass insert that the screw goes into has somehow broken free from the plastic and is turning as the screw is turned. If the screw is just knurled then it may be easy to break the bond, but if it is tapered or somehow keyed in that would be tougher. If the insert is the part that is square at the back as in the picture then I would say it is stripped not loose.
I'm leaning more toward the possability that the brass may be turning too ,hoping that with slight pressure pulled against adapter maybe it will back out ,I don't think they attempted to do this because the car was only on the rack a very short time.... ,,,SO HOPE IN ONE HAND CHECK BOOK IN OTHER
