It's definitely the fan control module. Same thing happened to my car few years back. Dealer replaced the module and the whole motor as that went bad too. This was a recall item so it's possible if it wasn't done on your car you'll get it done for free. Go to Chrysler.com and type in your VIN under the recalls section. It'll come up whether the replacement was done.
