It was the transmission - think of it as not fully "pushing the clutch in" when the car came to a stop. I remember he'd often put it in neutral while waiting for a light, then go to "D" and press the gas as soon as it "engaged". It was an old Pontiac, I think he sold that car rather than try to get it repaired, as it had many other problems leading up to that one.

Is this what you are describing? Or is it that the engine just isn't running right?
If this was the problem it would be evident when sitting in drive as the car would be fighting the brakes and the OP does not mention this. The RPM's would tend to be higher, as these cars will adjust to engine load to prevent stalling. So I'm guessing an engine management problem of some kind.
I'm guessing the idle speed is not being maintained properly. Vacuum? TPS? Something in the throttle system?
