1. high milage
2. defective bearing ( I had the tensioner bearing start squeaking on my 2002 Concorde, I injected some WD40 and it held out another month until I got around to replacing it )
3. excessive tensioner strength

Is this an NA or a SRT6 ? The load on the idler differs depending.
