Default Headlight Lense Inner Condensation

My "new" 2006 roadster has condensation inside the headlight lenses that tends to get worse in cool, wet weather. It builds up a "moisture film" on the inside of the lense that not only looks tacky, but can affect the headlight intensity. How does one remove the outer lense to clean the inside of them, and what could be causing the ingestion of moisture in the first place?
