Any evidence of water or oil leaks? OEM belt, or how many miles on the car or belt? This happened to me when pushing it hard at an autocross, but it turned out that I had a pin hole leak/crack on the return coolant line to the coolant reservoir. Actually it was at the plastic tee at the passenger side of the radiator. When it got hot, or tossed and torqued around(not sure)it apparently spritzed the belt enough to jump off when near WOT. I don't know if it jumped off approaching WOT, or upon the subsequent and sudden deceleration. Anyway.... No trouble since I replaced the plastic tee with a brass one. I was lucky not to sustain any damage what so ever, but could have been serious bad news! Take it easy until you get it figured out. Also, if you end up getting a new belt; placing the serpentine belt is super easy with a long breaker bar and torx socket for the belt tensioner(less than 5 minutes).
