This happened in my bosses car a few months back. Very early production crossfire. I assumed it was because he let everyone an there mother drive the vehicle. At least 60 different people.

The clip could have come loose, I'm not sure. He took it to the dealer and they swapped out / replaced the necessary parts. It is no longer an issue.
