Can you take some pics please?
Of the pads and rotor, so we can see how they are placed, might be alittle more help?
I have seen pads come unglued within a few miles after install.

Not much of a fan of slotted rotors, I like drilled alot, but slotted glazes the pads and greatly reduces life of pad, but they will stop good.
