The most common cause of the failure is the elements burn out... the actual cause of this is the control box is faulty and allows the elements to get too hot.
I have not yet had this happen to me, but I'm tempted to track down a 'newer' or 'replacement' part for the control box and just swap it out so I don't have the elements burn out.

It's usually the leads to the heaters getting pulled from someone using the seat as a prop or kneeing the seat bottom. A little soldering after removing the covers and getting to the wires fixes this.
