It happened in Madison county, just north of hwy 36 on 109 (near Emporia). I don't know if that's Anderson, Pendleton, or Markleville.
I can't read the officers signature, it looks kinda like D. S. Cl---- (scribble) with an ID # of 4810 (the zero might be a 6).
I'm supposed to appear in Elwood City Court on Aug 5th 2008 at 2pm.
And to add to what i said originally, it is in fact a summons.
