I sent them my complete boot, including the plastic mounting piece, so that they could create a template from it. They reattached mine to the plastic but if you order one now you'll have to gently pry the staples out of the existing piece and mount the new boot to it. That should be a very simple process though.

They have a wide selection of colors and materials, and at $35 it's a decent price for a high quality product.
