I have the same issue but when it goes into neutral it feels like someone stepped on the brakes real hard for a split second. I also have a 04 LTD. I have not had time to get the code checked due to the army keeping me busy but I just took 30 days off so I'm going to get it checked. I'll update as soon as I get it looked at.
