I did this about 2-3 yrs ago, so from memory:
Jack up car, remove wheel, remove a little bit of brake fluid from reservoir, depress caliper piston, remove caliper and hang securely out of the way, using pull hammer with claw attachment remove the, i think it's called the hub cover. use allen wrench to loosen the nut, then use wrench to remove the nut. Take note of the orientation of the bearings. The outer bearing should just pop out when you pull the rotor off. Use punch or big long screwdriver and hammer to punch out the inner bearing from the opposite side or use the pull hammer with attachment to pull out the inner bearing. You'll notice the seal that comes off with the inner bearing is not reusable. Make sure to clean out all the old grease. Then pack the new bearings in bearing grease. Now reverse the process. To get the inner bearing/seal to seat, light taps with hammer going around evenly. remember to pump the brake pedal and top off the brake fluid reservoir with fresh fluid.

I think that sums it up.
