My High Beams Are Gone

I just lost both my high beams and the blue high beam indicator light...has anyone had this happen to them? I am 1 month out of warrenty on my 2005 with only 24,000mi

I had the same thing happen to my car. These are the part numbers for the switch and lighting module they replaced under warranty 5098519AA and 5098638AA.
