Default Common SRT Belt Throwing Problems

A good friend owns an SRT and the acc belt doesn't want to run true. It shreaded a belt once before and tore the water pump pulley off.
The last time he had it running, the belt jumped one of the grooves, and it started to shread.
He said the last time he drove it, it felt as though the belt was slipping, at least that's the only way he could describe it.
He said he's running a 185 pulley. Does it have a history of causing this to happen?
Could the belt tensioner be the problem?
I don't think he's leaking oil from the oil filter O-Ring.
If it was the belt tensioner pulley going bad, I would think he would hear that.
Any other suggestions?
Thanks guys, I've never had the problem so I wasn't sure what to tell him. That, and I don't have an SRT.
