Have codes P2500 and C1026

Can somebody help me what does it mean!Realy need help!Because of this codes sometimes my car get in limp home mode.

I went to this site: OBD-II Trouble Codes - DTC Codes Car Repair
Can't find those codes. Are you sure they're right?
